ADMISSION POLICIES
Admission to St. Mary’s Academy of Carmen is granted with a definite understanding that the pupils, students and parents or guardian agree in writing to comply with the scholastic standards of the school and its rules and regulations.
The school reserves the right, whenever necessary and expedient, to treat violations of the school rules and regulations in accordance with the Christian system of discipline and guidance. The right to dismiss any pupil/student for poor scholastic work or school conduct unbecoming of a Marian is likewise reserved to the school authorities.
RE-ADMISSION POLICIES
The re-admission of students to the next school year is based on their academic and conduct performance in the previous school year.
Students who have been required to take subjects for two consecutive summers during their stay in high school may not be re-admitted.
Students who fail in conduct in any grading period will be placed under conduct probation and unless the probation status is lifted within the period specified in the agreement he/she may be dismissed from the school during or at the end of the school year.
Students who fail in one or two final ratings will be placed on academic probation the following school year.
Students who have been previously dismissed from the High School will not be re-admitted.
Students with failures and disciplinary problems must see the Prefect of Students’ Discipline, Academic Coordinator and the Principal before enrolment.
SMAC students who have transferred to another school but seek readmission are treated as new students. Therefore, they have to undergo the same procedure of getting application forms for admission, being interviewed and taking assessment examination.
PRE-KINDER, KINDERGARTEN & GRADE 1
Age Requirement:
Pre-Kindergarten - 4 yrs. old within School Year
Kindergarten - 5 yrs. old Within School Year
Grade 1 - 6 yrs. old within School Year
Photocopy of the BIRTH CERTIFICATE from the Philippine Statistics Authority or PSA (Original copy must be presented)
Photocopy of BAPTISMAL CERTIFICATE (Original Copy must be presented)
Four (4) I.D. pictures (1”x 1”)
Satisfactory rating in SMAC Assessment Examination and Interview
GRADE 2- 6
Photocopy of the BIRTH CERTIFICATE from the Philippine Statistics Authority or PSA (Original copy must be presented)
Certified copy of REPORT CARD (Form 138)
Certificate of Good Moral Character
Two (2) recent I.D. pictures (1”x1”)
A letter of Recommendation from the Principal/Guidance Counselor
KINDERGARTEN – Mental and Reading Ability Test (English and Filipino)
GRADE I – Satisfactory ratings in the written Mental Ability and Achievement Tests in Reading, Mathematics and Filipino.
GRADES 2-6 – Satisfactory ratings in the written Mental Ability and Achievement Test in Reading, Language, Filipino, Mathematics and Science.
Admission to the SMAC depends on the credentials presented, assessment examination results and interview of the applicant.
Requirements:
Photocopy of the BIRTH CERTIFICATE from the Philippine Statistics Authority or PSA (Original Copy must be presented)
Photocopy of BAPTISMAL CERTIFICATE
(Original Copy must be presented)
Report Card (Form 138) with no failing grades
Certificate of Good Moral Character
Four (4) 1”x 1” I.D. pictures
Medical Certificate
An academic and conduct average of 80% and/or above.
Satisfactory rating in SMAC Assessment Examination and Interview.
New Students/Transferees for Grade 9 and Grade 10 are accepted upon recommendation by the Principal/Guidance Counselor of the school last attended.
Foreigners/Aliens
Basic requirements stated above (a-i)
Alien Certificate of Registration (ACR)
Grade Placement from the Department of Education (DepEd)
Permit to study from the Embassy/Consulate of their respective countries.
ADMISSION PROCEDURE (For New Pupils/Students)
Initial Interview by the Academic Coordinator or the authorized representative.
Pay the testing fee at the Cashier’s Office.
Proceed to the Guidance Center and submit to the Testing In-charge for the (Assessment Examination).
Pupils/students must be at the testing room ten (10) minutes before the testing time.
Test results may be obtained on the date specified by the Guidance Center.
Pupils/students who pass the assessment examination may proceed to the Academic Coordinator for evaluation of exam result.
Upon the recommendation of the Academic Coordinator, pupils/students may proceed to the Registrar’s Office for the Registration form.
All transferees are considered Probationary Students (HS only). They shall proceed to the Prefect of Students’ Discipline and Academic Coordinator for signing of the Memorandum of Agreement.
Accomplish the Registration Form with the following requirements to the Registrar’s Office:
Form 138 (Report Card)
Baptismal and Birth Certificate (PSA Authenticated)
Certificate of Good Moral
Assessment Examination Result
Proceed to the Cashier for payment of fees.
Final interview by the Principal.
Back to the Registrar for the issuance of Class Admission Slip.
Note: Submit the admission slip to the class adviser on the first day of classes.
Old Pupils and Students
Present Form 138 (Report Card) to the Registrar
Accomplish the Registration Form.
Proceed to the Cashier for payment of fees.
Proceed to the Principal for signing of the Registration Form.
Submit the Registration Form to the Registrar and for the issuance of Class Admission Slip.
Note: Submit the admission slip to the Class Adviser on the first day of class.
